Once you reach the Garage in Resident Evil Requiem, you will find the Organ Transport Box. While you can interact with the item from your inventory, you will not be able to open without follow the specific sequence detailed in a note obtained by clearing the Lead Researcher’s Office puzzle.

The Puzzle Solution

The Unlocking the Organ Box diagram shows what you have to do to open the Organ Transport Box and in which order. If you're having trouble understanding what to do, perform the following actions while examining the item in your inventory:

  1. Rotate the dial from the Auto position to the Hand position.
  2. Move the vertical slider to a middle position.
  3. Toggle the fifth and second small switches from the left.
  4. Rotate the dial from Hand to Auto to open the box.

The Reward

Inside the box, you will find the Artificial Lungs. This is a critical puzzle item that connects directly to the Artificial Heart you may have already found. Both the lungs and the heart must be taken to the Examination Room and used on the Organless Corpse to obtain another critical item.
