This is the second case where a user reported damaged Ryzen 9800X3D a second time on the same motherboard. Not surprisingly, it was the ASRock motherboard that killed the processor.
Ryzen 9800X3D Killed by ASRock X870 RS Pro WiFi a Second Time Despite Using BIOS From May
ASRock "Murderboards" are doing their job of killing Ryzen 9800X3D (and other Ryzen 9000 processors) without failure. While a dead Ryzen 9800X3D isn't something new on ASRock motherboards, especially 800-series models, getting the CPU killed twice is now becoming common as well. If you remember, we reported an incident last month where one user got his 9800X3D killed twice on the same motherboard. Guess what? We have another user who reported the same.
Surely, the motherboard model is different in this case, but it's still from ASRock. User u/ShendonZ reported that he bought the Ryzen 7 9800X3D and the ASRock X870 RS Pro WiFi in the first half, but it only worked for around two and a half months before his 9800X3D died. Fortunately for him, he got a new CPU from AMD through RMA, but after using the second sample for the same time frame, the same incident repeated, and now he is without any working CPU.

He regrets not changing the motherboard, and the culprit is likely the ASRock motherboard, as has been seen previously. Unfortunately, the issue seems so complex that the root cause hasn't been found out even after months. Not just ASRock, but even independent reviewers couldn't solve this mystery. Gamers Nexus is still gathering all the reports and pieces for understanding the issue, and fortunately for this user, Steve has requested to buy both of his damaged CPUs and motherboard (Thanks, Steve!).

It's interesting to see that despite running a recent BIOS version 3.25 from May, the CPUs couldn't survive. It's also interesting to note that ASRock was the one who recommended this BIOS version, as it apparently fixed the "root" cause, which it claimed to be the PBO initially. The user says he didn't update to BIOS 3.30 or 3.40, as there were reports of issues with the new BIOSes as well.
What's worse is that ASRock guarantees that its motherboards aren't a problem, and it has "apparently" fixed the issue with the latest BIOS versions. It's not the first time it has claimed such, and it appears misleading since ASRock motherboards remain the number one killer of Ryzen 9800X3D CPUs to this day.
